York Region Public Health (YRPH) is conducting a survey to determine your experience from the service you received with Health Connection. We are inviting all participants to complete this survey. Your feedback will help us improve our programs to provide you with the best services.
This survey should take three minutes to complete. Participation in this survey is voluntary and your responses will remain anonymous and confidential. You may choose to withdraw from the survey at any time; however, we will not be able to remove your responses once you have submitted the survey. Your decision to participate or not participate will not affect the services you receive from YRPH.
